      <abstract>
        <p>Benign Prostatic Hyperplasia (BPH) is one of the most prevalent urological conditions affecting ageing men. It is characterised by non-malignant proliferation of prostatic glandular and stromal tissues, frequently resulting in lower urinary tract symptoms (LUTS), including increased urinary frequency, nocturia, hesitancy, weak urinary stream and incomplete bladder emptying. Conventional management includes pharmacological interventions and surgical procedures, depending on disease severity and patient characteristics.
Ayurveda describes urinary disorders under the broad spectrum of Mutraghata, with Vatashthila being a condition that demonstrates clinical similarities to certain manifestations of BPH. Uttarbasti, a specialised Ayurvedic para-surgical procedure involving the administration of medicated preparations through the urethral route, offers an interesting area for clinical investigation in Ayurvedic urology.
This short communication discusses the Ayurvedic rationale, potential therapeutic relevance and clinical considerations of Uttarbasti in the management of BPH. It also highlights the need for standardised protocols, objective outcome assessment and well-designed randomised controlled clinical trials to establish its safety and therapeutic efficacy.
